INSTRUCTIONS FOR OUR NEW ENTRY BLANK:
Please read carefully before submitting.
1. Download Entry Blank. NOTE: There is an icon in the Adobe Reader toolbar that looks like an old floppy disk, this allows you to save the file to your computer.
2. Fill in all appropriate information. (Do not worry if you cannot digitally sign the file)
3. In addition to emailing/faxing/mailing your completed form, print and sign a hard-copy, and bring it with you the day of the show.
4. Save filled out form under this naming convention:
MM-DD-YY_HorseName.pdf
Example: 12-06-09_RollingThunder.pdf
NOTE: Exact dates for horse shows are available on our website.
5. You have 3 options for submitting your form:
5.a) Email completed form to: handsonhorsemassage@yahoo.com as an attachment. NOTE: Please use the naming convention from the previous step as your subject line.

5.b) Or: Print your completed form, and Fax it to (631) 239-1732

The 2010 Grand Champion Awards Dinner Dance will be held
May 21st, 2010 at the Savoy Room at the Huntington Hilton!

Thomas School of Horsemanship calculates high point winners for the Thomas School Show Series each year. Highest point winners receive awards at our Annual Dinner Dance, including "The Stormy Weather Perpetual Trophy" awarded to the highest overall point winner, and "Thomas School Rider of the Year" awarded to the TSH student demonstrating the highest excellence in both sportsmanship and competition. Grand Championship winners will receive an award, while Reserve through 6th place will receive year end ribbons.
