INSTRUCTIONS FOR OUR NEW ENTRY BLANK:
Please read carefully before submitting.
USEF Show Entry Blank*
1. Download Entry Blank. NOTE: There is an icon in the Adobe Reader toolbar that looks like an old floppy disk, this allows you to save the file to your computer.
2. Fill in all appropriate information. (Do not worry if you cannot digitally sign the file)
3. In addition to emailing/faxing/mailing your completed form, print and sign a hard-copy, and bring it with you the day of the show.
4. Save filled out form under this naming convention:
MM-DD-YY_HorseName.pdf
Example: 12-06-09_RollingThunder.pdf
NOTE: Exact dates for horse shows are available on our website.
5. You have 3 options for submitting your form:
5a) Email completed form to: handsonhorsemassage@yahoo.com as an attachment. NOTE: Please use the naming convention from the previous step as your subject line.
Example subject line: 12-06-09_RollingThunder
5b) Or: Print your completed form, and Fax it to (631) 239-1732
5c) Or: Print your completed form, and Mail entries to:

6. On the morning of the show please be sure to bring:
i. A printed hard-copy of your completed Entry Blank,
ii. Check or Cash,
